The cardiac silhouette is obviously enlarged. Note that a region of the left heart touches the chest wall. What chamber or portion of a chamber is this?

There is a region of soft tissue or fluid density below the last thoracic vertebrae that is due to pleural effusion. There is also a region of the cardiac silhouette that appears denser than the rest of the silhouette. This region corresponds to the region that touches the chest wall on the DV view. T
